Terms 

Functional Therapy is the application of science in the promotion of health, peak performance and individual care. Functional practitioners use a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual's symptoms and health concerns. Functional therapy is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. 

Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach. Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs or symptoms to their own GP. They will also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client's care to explain any therapy programme that has been provided. 

The Functional Practitioner requests that the Client notes the following - 

1)The degree of benefit obtainable from Functional Therapy may vary between clients with similar health problems and following a similar Therapy programme. 

2) Functional advice will be tailored to support health conditions and/or health concerns identified and agreed between both parties. 

3) Functional advice is not a substitute for professional medical advice and/or treatment. 

4) Your Functional Practitioner may recommend food supplements and/or functional testing as part of your Therapy programme and may receive a commission on these products or services.

5) This document only covers the practice of Functional Therapy within this consultation, and your practitioner will make it clear if he or she intends to step outside this boundary. 

The Client understands and agrees to the following – 

7) I am responsible for contacting my GP about any health concerns. 

8) I give permission for you to contact my GP regarding any agreed aspects of my case.

9) If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any Functional strategy provided by my Functional practitioner. This is necessary because of any possible reaction between medication and the programme. 

10) It is important that I tell my Functional Practitioner about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the programme. 

11) If I am unclear about the agreed functional therapy programme / food supplement doses / time period, I should contact my practitioner promptly for clarification. 

12) I must contact my practitioner should I wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions. 

13) Recording consultations using any form of electronic media is not allowed without the written permission of both me and my Functional Practitioner 

CANCELLATIONS 

If you need to cancel or change an appointment, 24 hours notice is required. If less than 24 hours notice is given, the appointment will still be chargeable and will count as a lost session in a package 

RE-SCHEDULING

Sessions may be re-scheduled upon 24 hours notice by contacting Dr Gayeyri's Clinic, preferably via your patient portal, but also on 07526878049 or consult@drgayetri.co.uk 

TIMELINESS 

Please be on time for your appointment, if you are late your session may be cut short. If you know you are running late please contact me on 07526878049 and we will decide how best to go forward with the remaining time. In some cases, the session may be forfeited. 

OTHER CONTACT 

Please try to keep the majority of contact via the Patient portal. You can do this online or through the app for your smartphone. This is because emails are easily overlooked or lost or end up in a spam folder. Patient offers a secure and easy way for me to manage client contact and records in a secure and organised manner. 

Between appointments, one email or telephone conversation which takes less than 10 minutes to deal with will not be charged for on top of your program fee structure. If however, you wish to discuss any complex matters regarding your program or your progress, you will be charged for on a pro-rata basis of the hourly £100 appointment time. 

Any quick questions / answers will not be charged for. 

The exception to this is Health Coaching programs where more frequent contact has already been agreed and built into the program fees. 

PROGRAMME DURATION 

You get best results if you work consistently and steadily towards your goal. With that in mind, 3 month (12 week) packages have an expiry date 4 months (16 weeks) from the Initial Consultation session. And 6 month (24 week) packages have an expiry date 7 months (28 weeks) from the Initial Consultation session.


This allows for flexibility to work around holidays and other contingencies but still helps you keep focused, accountable and on track. Please note that packages do have an expiry date. 

Sessions skipped during this time and not taken before the expiry date are not refunded. 

PAYMENT 

All Services must be paid for in full by BACs/bank transfer or card payment at least 24 hours in advance of the initial consultation appointment. 

REFUNDS 

If you have commenced a programme and wish to cancel prior to the end of all scheduled services offered within the programme, you shall be obligated to pay all fees accrued prior to the effectiveness of your cancellation. Dr Gayetri reserves the right to include fees for all preparatory work and research undertaken in addition to the scheduled contact dates for consultations and calls. 

Dr Gayetri Clinic reserves the right to decline to provide the Services to you for any reason and will refund any amounts pre-paid by you for Services not received. 

OTHER COSTS 

The cost of supplements and tests are not included in consultation fees
